Our experienced urgent care healthcare providers are fast, friendly, and can get you the help you need efficiently. Got a cut that needs sutures? We can do that. Maybe a little state-of-the-art surgical glue on that cut instead? Ooh, now we’re talking. Oh, and by the way: you can make an appointment if you’d like (online, by phone, or even by fax), but appointments are never required, and our wait times are often less than 30 minutes per patient. Get your Illinois Medical Card now from a doctor’s clinic that you can trust! Our patients can now use their own medical insurance to be patients at our primary care clinic. After two visits, the physician can recommend you for the Illinois Medical Card.
There are 2 Programs in Illinois (OAPP and MCPP). Both are different, but OAPP is for 3 months and MCPP is for 1-3 years. Also, qualifications are very different. We recommend MCPP if you have a qualifying condition on the condition list and OAPP if you do not have one of those qualifying conditions (but also get prescribed opiates):

If you have these three qualifications, you can get your Illinois medical card. The application cost is $10, and you are certified for 3 months, after which you will need to make another appointment to get re-certified.
*A condition that warrants an opiate includes migraines, chronic neck or back pain, trigeminal neuralgia, intractable pain, neuropathic pain, musculoskeletal pain, pelvic pain, endometriosis, joint pains, shoulder pain, post-operative pain, etc. But, you must have medical records showing you have seen a doctor for this condition and you have this condition (we do not make these diagnoses for you unless you see our primary care providers).
**Bona fide patient-physician relationships mean that you must see us at least twice to get your medical card. You do not already need a bona fide patient-physician relationship (you can create that relationship), we just want to be clear that we’ll have to see you twice in order to get your card.
*If we do not take your insurance, you qualify for the Self Pay Plan, which is $175 per visit. Normally, a patient-physician relationship takes at least 2 visits.

The total cost to get an Illinois Medical Card here in Illinois depends on your insurance, but for a 1 year application not including doctor’s fees, the total cost is: $102.35 ($102.35 for Illinois certification for 1 year with their processing fee) or if you are on disability or are a veteran, the total is $51.18 ($51.18 for Illinois 1 year certification with processing fee). To our knowledge, we are the only office that takes insurance for your physician visits.
By the way, this is a one-stop-shop for you to get your application sent to the Illinois Department of Public Health. It is also the most cost efficient. WE TAKE MOST COMMERCIAL INSURANCES for OAPP and MCPP.
