No appointment needed. Simply submit a form to tell us what’s going on and we’ll be in touch. This service is self-pay only, for about the same cost as a typical co-pay.

What started as one single urgent care has grown to become a full medical group serving the Northside of Chicago. Innovative Care offers primary care, urgent care and therapy (behavioral health) services to our patients. Together, this covers 90% of most patient’s ongoing outpatient healthcare needs.
